SUPPORTIVE FAMILY CONNECTIONS PROGRAM

The Supportive Family Connections Program is a wraparound case management and family stabilization service designed to support parents, caregivers, and families navigating complex life circumstances—especially those involved with Child Welfare, court-ordered reunification processes, and community-based service systems.

At Supportive Equity Connections of Tampa Bay, we believe that every family deserves access to compassionate support, practical resources, and a clear pathway toward healing and stability. Our program is rooted in dignity, equity, and empowerment, ensuring that families are not defined by their challenges, but strengthened through meaningful connection and guidance.

A Family-Centered Approach

Supportive Family Connections provides individualized care coordination for adults who may be facing barriers such as:

  • Child Welfare involvement or reunification court requirements

  • Housing or financial instability

  • Emotional or developmental challenges

  • Limited support systems

  • Difficulty navigating social service systems

We walk alongside families to help reduce stress, build protective supports, and promote long-term success.

How We Support Families

Through intentional case management and advocacy, participants receive assistance with:

  • Understanding and meeting case plan or court-ordered requirements

  • Referrals to parenting education, counseling, and supportive services

  • Connection to housing, employment, transportation, and basic needs resources

  • Emotional support, accountability, and goal planning

  • Coordinated communication with service providers when appropriate
