The Supportive Family Connections Program

The Supportive Family Connections Program is a wraparound case management, family stabilization, and senior support service designed to assist individuals and families navigating complex life circumstances. The program serves parents, caregivers, families, and adults age 55 and older, including individuals involved with Child Welfare, court-ordered reunification processes, aging and senior service systems, and other community-based support systems.

At Supportive Equity Connections of Tampa Bay, we believe every individual and family deserves access to compassionate support, practical resources, and a clear pathway toward stability, independence, and an improved quality of life. Our program is rooted in dignity, equity, connection, and empowerment—ensuring that individuals are not defined by their challenges or their age, but supported in building upon their strengths.

An Individual and Family-Centered Approach

Supportive Family Connections provides individualized care coordination and support for adults and families who may be facing barriers such as:

• Child Welfare involvement or reunification court requirements

• Housing or financial instability

• Emotional, developmental, or life-transition challenges

• Limited family or community support systems

• Difficulty navigating social service, healthcare, or government systems

• Social isolation or loneliness

• Challenges associated with aging or maintaining independence

• Transportation and mobility barriers

• Food insecurity or difficulty accessing basic necessities

• Questions or challenges related to senior benefits and community resources

• Caregiver support needs

We walk alongside individuals and families to help reduce stress, strengthen natural and community supports, increase access to resources, and promote long-term stability and independence.

How We Support Individuals and Families

Through intentional case management, advocacy, resource navigation, and ongoing support, participants may receive assistance with:

• Understanding and meeting case plan or court-ordered requirements

• Referrals to parenting education, counseling, and supportive services

• Connection to housing, employment, transportation, food, and basic-needs resources

• Assistance identifying senior services, benefits, and community resources

• Connecting adults 55+ with programs that support independent living and healthy aging

• Emotional support, accountability, and individualized goal planning

• Reducing social isolation through connections to community programs and activities

• Coordinated communication with service providers when appropriate

• Identifying healthcare, wellness, and behavioral health resources

• Caregiver resources and support

• Advocacy and assistance navigating complex service systems

Supporting Adults 55+ With Dignity and Connection

For adults age 55 and older, Supportive Family Connections recognizes that aging can bring changing needs, unexpected transitions, and new challenges. Some older adults may find themselves navigating housing concerns, fixed incomes, transportation barriers, changing family dynamics, caregiving responsibilities, or complicated service systems without adequate support.

Our goal is to help seniors remain connected, informed, supported, and as independent as possible.

Rather than expecting older adults to navigate multiple agencies and systems alone, SEC helps identify needs, connect participants with appropriate resources, coordinate services when necessary, and provide consistent support throughout the process.

The goal of Supportive Family Connections is to strengthen individuals and families from the inside out—supporting parents working toward stability and reunification, caregivers supporting loved ones, and older adults seeking to maintain independence, security, and meaningful connections within their communities.
